Appointments

When you call ahead to make an appointment, the Pareja Medical Center staff needs your insurance information so that we can verify your coverage prior to the first visit. You are asked to arrive a half hour early to fill out all documents. If you come with the paperwork completed, then please arrive fifteen minutes ahead of your scheduled appointment. Pareja Medical Center attempts to give you a courtesy call in order to confirm scheduled appointments 24-48 hours in advance. There is a $25 fee for missed appointments or late cancellations. Please call to cancel appointments at least 24 hours in advance. Walk-in patients are accepted, but will be attended to after previously scheduled patients.
Insurance
Pareja Medical Center accepts PPO and Medicare insurances. Medicaid is accepted as a secondary insurance to Medicare. A sample of plans accepted by Pareja Medical Center is found below. If your plan is not on this list, please call so that we can obtain more information and verify your insurance. Co-pays and deductibles are due at the time of service. We also welcome patients who are not insured but will render payment in cash. Because we at Pareja Medical Center want everyone to achieve an ideal state of health, we attempt to make our prices for all of our patients as affordable as possible regardless of their financial situation.

Cosmetic Services
An initial consultation with the medical doctor is necessary before proceeding with any kind of laser treatment. At that consultation, you will be informed of the cost of your treatment. Because each person’s needs are different, prices cannot be posted on this website. Please call our office for more information. All cosmetic services are payable only with cash, and the entire series of treatments must be paid at your first treatment (usually a few days or a week after the initial consultation). Physicals / Immunizations
Limited appointments for school and sports physical and immunizations are available. Parents or guardians of a child must come prepared with all documents filled out. If the provider has to fill out the paper work for you, you will be charged more for his time.
The only immunization administered at Pareja Medical Center is tetanus.
Financing and Billing
If you are a current patient and have a question about a recent statement, please call our office or call our billing line directly: (888) 385-8767. If you are a new patient, only initiate contact with our office.
Certain services, and all non-traditional medicines, will not be covered by you health insurance company. For these services we have priced our rates with your health and well-being in mind so that you can afford to achieve your ideal state of health. As always, we maintain very reasonable cash pay rates at Pareja Medical Center so that people without health insurance coverage can afford to obtain the necessary treatment needed for their health needs.

Dr. Pareja prefers to see patients in his office over phone consultations. Please call to schedule an appointment.
Seminars/Conferences
Dr. Pareja is available to give seminars or participate in conferences or symposia. Please call the office or email at least 4-6 weeks in anticipation of the event to discuss the possibilities. |
